With Children’s Complex Care now part of the wider Brightpath Healthcare specialist group, we are further strengthening our commitment to delivering safe, consistent and personalised care at home, where individuals feel most comfortable, while upholding the highest standards of clinical excellence. Collaborative working
Due to the nature of complex conditions, individuals may have multiple professionals involved in the support they receive. We strongly believe that collaborative working is essential to delivering effective care. That is why we work closely with everyone involved in each individual’s care programme. By working together, we ensure clear communication and a coordinated approach, enabling consistent, specialist support for children, young people and adults.
Our Healthcare Assistants benefit from ongoing nurse-led support and mentoring, ensuring they can confidently and safely support the complex needs of children, young people and adults.
Our Clinical Nurse Managers oversee the delivery of clinical care, ensuring it meets best practice quality standards and supports positive outcomes for every individual. By training our Healthcare Assistants to perform clinical duties we can reduce costs while delivering hospital-grade care and support.
We ensure our care is holistic, addressing all aspects of an individual’s needs. Collaborative working is central to this approach, which is why we work closely with the wider network of healthcare professionals involved in each person’s support. Together, we form a multidisciplinary team that works in a coordinated way to support every aspect of complex care for children, young people and adults.
Living with complex care needs often means that individuals may have multiple professionals involved in coordinating and monitoring their care and support. We recognise that this can feel daunting, so we work closely with you and other professionals – including solicitors, case managers and social workers – to ensure the process is clear and well coordinated.
We also connect with schools and community-based organisations to ensure children, young people and adults have access to meaningful opportunities that support their development, independence and overall quality of life.
